With a few nice punches and some 'kiting' you basically could kill a good 80 percent with lots of ammo to spare. The game is very forgiving and I literally never used medkits instead I just save them for the passengers.Personally, I don't like the tedious mini-games on the train because it distracts me from the conversations between passengers. The lore was okay but leaves me really thinking at the end.
